The following forms are required by the state of California for each child to enroll in Linden Community Preschool and need to be turned in before your child can attend.
All can be downloaded to your computer or tablet, filled in online and printed for signature. If you are unable to download and print the files please call and schedule a visit (209) 887-3253 to pick up a packet.
